What We Do:Minimally-Invasive Surgery

Rather than the large incisions of traditional surgery that take longer to heal, we perform minimally-invasive laparoscopic surgeries through small incisions only 5mm (¼”) long. All surgeries are performed as outpatient procedures.

Shimbashi DAY Clinic is conveniently located just a 1-minute walk from JR Shimbashi Station, approximately 30 minutes from Haneda Airport, and around 60 minutes from Narita Airport. Hernia Surgical Repair & Techniques
At Shimbashi DAY Clinic

Inguinal hernia is the most common type of hernia. This type may occur in one or both sides of the body and is common in men than women.
At Shinbashi DAY Clinic, patients can complete their treatments in a comfortable environment with minimally invasive laparoscopic surgeries performed by specialized surgeons, and general anesthesia administered by anesthesiologists. During the surgery, anesthesiologists provide a nerve block injection in the abdominal area, significantly reducing post-operative pain.

Outpatient Surgery

Outpatient surgery allows the patient to undergo a procedure without hospitalization and return home on the same day. The time spent in the clinic is minimized, enabling the patient to resume their normal routine quickly. This approach has minimal impact on work commitments and household responsibilities. As the duration of the clinic stay is short, the overall costs are also relatively lower. Patients can recover in the comfort of their own homes, reducing the psychological stress often associated with hospital stays. Such treatment is especially beneficial for working individuals, parents of young children, and international travelers (Short-term visitors to Japan).

Minimally-Invasive Surgery

Minimally invasive surgery (MIS) is a contemporary surgical technique that uses small incisions instead of large ones typical in traditional surgeries. Through these incisions, surgeons insert specialized instruments and cameras to guide the procedure. Benefits of MIS include reduced scarring, less postoperative pain, quicker recovery, and a lower risk of infection. Many procedures, from gallbladder removal to hernia repairs, now utilize this approach. Specialized training is essential for surgeons to effectively perform MIS, as they operate based on camera-generated images. In short, MIS offers patients a safer, faster-recovery surgical option with minimal scarring.

Pain relief

For laparoscopic surgeries, conduction anesthesia is essential to enhance patient comfort. While general anesthesia is the primary method, our clinic combines it with a block injection to optimize post-operative pain relief. This block injection, applied at two specific sites on each side of the abdomen, disrupts pain signals from the surgical sites and peritoneum to the brain. Levobupivacaine is the local anesthetic of choice for this, providing pain relief for an average of nine hours post-surgery. Its ability to remain in targeted body compartments ensures prolonged pain relief from minimal doses. In contrast, anesthetics like lidocaine and bupivacaine, which last approximately 2-3 hours, are not the primary choices for post-operative pain management in this context.

General Anesthesia

Laparoscopic surgery for inguinal hernia mandates general anesthesia. In the operating room, patients are connected to monitors and provided oxygen through a mask. An intravenous drug induces unconsciousness, and a laryngeal mask assists breathing. Patients don’t recall the surgery upon awakening. Brainwave monitoring ensures optimal anesthesia depth. To allow same-day discharge, inhalation anesthetics are avoided in favor of intravenous ones. Muscle relaxants are counteracted post-surgery with sugammadex, ensuring no lingering effects. Only ultra-short-acting analgesics, such as remifentanil, are used, avoiding post-operative narcotic residues.

Evaluation

A hernia specialist will evaluate your condition to decide if surgery is warranted. Visitors to Japan on a short-term basis, including medical tourists, can have their pre-operative consultations online. Furthermore, for patients previously diagnosed with an inguinal hernia who have a referral letter, it's possible to arrange the surgery date prior to the face-to-face consultation.

02. Preoperative Examination

You will be required to undergo a series of tests including blood work, an electrocardiogram (EKG), and pulmonary function testing. For patients with a referral letter who have already had any of these tests performed at the referring medical institution, we will not repeat these tests at our clinic to avoid duplication.

03. Hernia
Surgery Day

On surgery day, we'll treat your hernia with a minimally invasive laparoscopic procedure. The surgery itself takes around 30 minutes, with the total expected time at the clinic being about four hours, which includes your recovery period. You'll be able to head back to your home or hotel on the same day to begin your recovery.

04. Post-Op Evaluation

Approximately one week after your surgery, we will schedule a follow-up visit to check on your recovery. This can be conducted online. While a face-to-face appointment is not usually necessary, we are also happy to accommodate in-person visits at our clinic if that is your preference.

With minimally invasive laparoscopic surgery and specialized anesthesia tailored for outpatient procedures, we can treat inguinal hernias in about four hours, allowing patients to return home the same day. This quick turnaround is especially appreciated by those on a short-term visit, as it minimally impacts their other schedules.

  • You may shower starting from the day of surgery.
  • Bathing in a bathtub is permitted from the next day onwards.
  • There is no need to disinfect the wound.
  • There are no dietary restrictions after the surgery. You may consume alcohol starting from the day after the surgery.
  • You can return to work or school the day after surgery.
  • While there are no restrictions on general activities such as travel and going out, vigorous exercise and lifting heavy objects are prohibited for two weeks following the surgery.

After completing his clinical fellowship at the University of Strasbourg’s IRCAD (Institute for Research into Cancer of the Digestive System) and EITS (European Institute of Telesurgery) in France, he returned to Japan. Subsequently, he held various positions, including the Head of Day Surgery at Yotsuya Medical Cube, Chief of Surgery at Itabashi Chuo General Hospital, and Director of the Laparoscopic Surgery Center, before taking on his current role. He has successfully performed over 1,000 surgeries and has spearheaded numerous laparoscopic surgery workshops in both France and Japan. With a deep expertise in diagnosing and treating abdominal hernia conditions, including inguinal hernias, he places a strong emphasis on practical clinical guidance and fostering the next generation of surgeons. Additionally, he holds a pivotal role in the academic organization ‘Tokyo Hernia Academy.’

After graduating from the National University of Yamanashi, he received training in general anesthesia, cardiac surgery anesthesia, and intensive care at both Yokohama City University and the National Cardiovascular Research Center. Following his service as an anesthesiologist and intensive care physician in hospitals with large-scale operating rooms, he founded the Shinbashi DAY Clinic in April 2022 and assumed the position of director. Since 2020, through his involvement in anesthesia for outpatient surgeries at specialized facilities, he has deepened his appreciation for the essential role that anesthesiologists play in day surgeries. Consequently, he has been passionately educating the upcoming generation of anesthesiologists on this critical aspect.
